I have found that the "Are you really sure?" popup only seems to appear if I click the correct chest. If I choose the wrong chest I get no popup and the chest simply opens. This isn't a big deal but it does seem a bit odd.

It's pure coincidence. I've gotten plenty of 'are you sure' messages on an empty chest.

Either way it seems weird that it's inconsistent. Shouldn't it either always pop up or never?

No, it's a random chance of getting the pop-up after clicking, regardless of what's in the chest. It's been like that since the beginning of the Treasure Hunt.

Just to be clear, this is not like Safe Mode. Safe Mode is to prevent you from releasing pokémon [shinies] or adopting eggs [chain breaking]. There is no such consequence to the Treasure Hunt, the pop-up is just to mess with you a little.
